Empire Legacy HealthCare Inc. provides person-centered services and supports for adults 21 years of age and older with intellectual and developmental disabilities (IDD). We recognize and value each person’s unique abilities, interests, experiences, and aspirations.  Our services are guided by each individual’s Person-Centered Plan (PCP), with a focus on their identified needs, strengths, and personal goals. We promote dignity, respect, self-advocacy, privacy, health and safety, and informed decision-making. Through individualized support, we help individuals develop daily living skills, build confidence, strengthen meaningful relationships, pursue opportunities, and participate actively in their communities.

Supported Living Services

Our Supported Living Services help individuals live as independently as possible while receiving personalized assistance based on their needs, preferences, and Person-Centered Plan (PCP). We support individuals in developing daily living skills, managing household responsibilities, making informed choices, and participating actively in their communities.

Community Development Services

Our Community Development Services (CDS) provide opportunities for individuals to build practical skills, strengthen social connections, and participate meaningfully in their communities. Supports are based on each individual’s interests, abilities, goals, and Person-Centered Plan (PCP), with a focus on greater independence and community inclusion.

Community Participation

We empower Individuals to develop essential life skills, build meaningful relationships, and actively participate in their communities. Guided by each Individual’s interests, goals, and Person-Centered Plan (PCP), our services promote independence, confidence, personal growth, and community inclusion.

Group Home Support Services

Our Community Living Group Homes offer safe, supportive, and family-oriented residential settings where individuals receive personalized assistance while developing greater independence. Supports are guided by each individual’s needs, preferences, goals, and Person-Centered Plan (PCP), with opportunities to build daily living skills and participate meaningfully in community life.

PERSONAL SUPPORT SERVICES

Our Personal Supports services help Individuals maintain independence, develop essential life skills, and participate meaningfully in their homes and communities. Our trained Direct Support Professionals (DSPs) provide individualized assistance based on each Individual’s needs, preferences, abilities, goals, and Person-Centered Plan (PCP). Support focuses on helping Individuals make choices, strengthen their skills, maintain daily routines, and achieve greater independence while receiving the level of assistance that is right for them.

Nursing Support Services

We promote health, safety, wellness, and independence through individualized care, health education, ongoing monitoring, and coordination of healthcare needs. Supports are aligned with each individual’s Person-Centered Plan (PCP) to promote well-being, address health needs, and reduce preventable health complications.

Promoting Healthy Relationship

We promote healthy relationships built on trust, respect, and meaningful connections. Community engagement creates opportunities for active participation and personal growth. Inclusion and belonging help Individuals build lasting relationships within their communities.

Respite Care Services

We offer short-term support for individuals 21 and older with Intellectual and developmental disabilities (IDD), giving unpaid caregivers a temporary break. Services are available for those enrolled in Maryland DDA HCBS waivers, including Community Pathways and Community Supports.

Employment Services

We support individuals in pursuing meaningful, competitive employment based on their interests, strengths, and career goals. Services may include job readiness, career exploration, skills development, job placement, and ongoing workplace support, promoting greater independence and long-term employment success.

CommunityIntegration

Individuals are supported to participate in community activities based on their interests and choices. Opportunities for social interaction encourage friendships, confidence, and meaningful connections. Community integration promotes inclusion, independence, and a strong sense of belonging.
